Need help on "how to write a formal complain"?


Someone at work verbally attacked me last week and I would like to write a formal complain to stay in his personal records. I've never written one before, So if anyone can help me, I would really appreciate it!

Dear employer

On [date], [who] said [what] which I consider offensive and bullying [or whatever]

For your information [who] was present and may be able to give you further information.

I expect you to protect me from such behaviour on the part of your other employees.

Yours sincerely.
