How can an RN get back into hospital nursing if out for 5 years?

It's usually not a big issue. If you've had previous experience, that's all that matters. As long as your license has been kept current, there should be nothing standing in your way.

At interviews, just be honest about why you left hospital nursing and what is driving you to want to return to it.

No matter how experienced you are, every hospital is different and will have an extensive training program before you are on your own.

Call your local licensing board and ask them what you need to do. If you're still licensed, apply for jobs and be honest with the hospitals. Tell them that you need some re-training.
