Whats does a Social Worker, the Nursing Home does?



This i have to get a reply by 20/8/07 cuz tmr i nid to pass up

Admissions, discharge planning, moving patients to different rooms if necessary, provide and refer out for psych services, MDS (assessment of resident's psychosocial status), care plans (how psychosocial issues are to be addressed), take and resolve complaints from patients and families, supervise the activities program (sometimes), spend an ungodly amount of time in meetings... Although your question is not clear I suggest the Social Worker attends to the external needs of the Nursing Home patients (Finance Placing Etc). The care staff look after the internal needs (nursing etc.)
Discharge planning, assisting residents/families with public aid applications (i.e., Medicaid/Medi-Cal), conduct admission tours, screen applicants for admission, and interface with social workers in other settings (hospitals, public agencies and hospice).
I worked in nursing homes for several years.
