Do potential employers call the hr dept of your old job or just the references that you put down?

Depends on the employer. Some employers call neither, and some call all. The HR Departments are very limited by privacy laws as to what they can tell future employers though. Mostly, they can just verify employment. Actually your references can tell more than your old job's HR Department! You choose the references though, so they should give you shining reviews.
Most places will call your references directly. If the companies your references work for (assuming they still work for the same company you did) have a policy against anyone but HR giving references (a lot of companies these days do,) the reference will likely refer them to HR. Most HR depts. will only verify dates of employment, last job title, and eligibility for rehire.

I always call both as no one ever lists their bad references.

Plus, the only way you can verify titles, time worked and salary is through the HR department at previous companies. I use the department to make sure applicants are not lying to me about their backgrounds.

Personal references are all taken with a grain of salt, most people I know would lie for a friend.

We actually do both. It really depends on the job classifications, duties, etc. What most people tend to forget is that we prefer that professional references are given, especially those that are of superior/managerial status. If all references are only peers, family, or friends, then of course, the HR department is contacted. Keep in mind that only certain questions may be asked - salary information is not provided unless you have signed an allowance form of some sort beforehand allowing that information to be discussed.
