Is it standard practice to interview for a job that has already been filled internally?

Yes - many managers want to have a choice of candidates from which to select, even if an internal candidate is the favorite. Others want the process to appear unbiased, even though it is. Still others may be looking for outside candidates with better skills so they don't get the boss's nephew foisted on them.

Take a shot and interview anyway - you may be able to show up the internal candidate (yes, it does happen!) If nothing else you'll get some practice at interviewing.

Normally if the position is filled an employer should save people the time of interviewing. Sometimes however there are other, unadvertised positions that they think you would be good for so having done the work of picking out your resume they will continue. If you seem like the right person to hire they will tell you about the position they are really considering you for.
So you shouldn't be upset if you are interviewed and later find out the position was already filled.
Yes, it happened to me. Finally after two interviews the person giving the interview was nice enough to tell me that it was pretty much a 100% chance that it would be filled internally. This saved me an untold amount of money in travel expenses since I was traveling an hour each way. By the way it was a government job so they had to open the interview process up to the general public. What they did was illegal but what the hell, it only paid $15,000 a year. I make three times that with the business I started on my own. They did me a favor.
Yes. I have been to interviews where I have had the feeling that the interviewers were just going through the motions, which made me wonder if they already had someone in mind. It has been done in companies where I worked, too. A niece of the manager or a former employee of the manager from her previous workplace would be employed after the lengthy interview period was completed.
