You may log-in to https://sss-online.sss.gov.ph/sss/contro... You just need to enter your SSS #, Lastname and Birthdate. Once you are in the system, you can view your contributions to date by clicking on Actual Premiums on the upper left side of the screen. If you want a print out, you can just copy+paste it in a Word Document. Hope this helps!
