Should you ever attach a photo of yourself with an online job application?

No, if you are sending your application in an email, most email filters will see the .jpg file and black list your email. You need to impress the company with your resume and qualifications not your looks.
I would *never* do this (unless, of course, the job post requested a photo).

Sending in a photo can be misinterpreted... employers will wonder why you sent in your photo. Are you trying to trap them, claiming prejudice if you're a minority and you don't get the job, or inequality if you're female, or if you're good looking, are you trying to get the job based on your looks? Maybe age discrimination will be the issue. Whatever positive reason you might have can easily be destroyed by the negative questions that will arise.

Stick to the written directions on the job post. You can't go wrong.
Absoluty not. There is no reason to. I personally would feel it's unprofessional.
Not in the US. It actually makes it more difficult for the employer wince viewing a photo before inviting the applicant for an interview (or not) can open them to charges of discrimination. Same goes for "video resumes" - best to skip it and let your experience speak for itself.
