What is the proper way to reveal in a job interview that I have surgery coming up in the next few weeks?

Do I even tell the interviewer this?! Help, please!

Don't say anything until an offer is made. Once there's an offer on the table, then you tell them, "I have a procedure scheduled for <insert date> and the doctor thinks I will be back to normal in <time frame>. I feel it's fair to let you know right away, so if we need to move back my start date we can."
The first answer is absolutely correct. I've been in HR over 10 years. Do not divulge the information until after you have an offer. Then simply state, "Yes, I'd love to take the job. I do have to tell you I have a surgery scheduled xyz, so I would not be able to start the position until xyz".
