That's pretty good. You have the relevant info, the contact info, you also have the relevant details to enhance your skills and achievements. That's good. But are you going to do this for every job you have had?

yes that sounds very good. it gives a good idea of what your skills are.
This is OK, I would add what you can bring to the new company, why you are better than somebody else. Good Luck, you have a good profession,
This is just very quick, and I am not sure if the terminology fits the industry.

* Prepared quotes for customized outdoor living areas. Quotes included tile, decking, fencing, as well as the concrete.
*Negotiated job costs and specifications to achieve superior results on all projects.
* Interacted with customers throughout the project to insure customer satisfaction.
* Prepped a variety of concrete jobs, including house slabs, factory work and driveways.
* Proficient in all stages of concrete work including finishing and sealing.

A few questions. Are you looking for the same type of job? If so, use the techinical terms for prep, poar, and finishing so you sound like you really know it. When you quoted jobs, did you need to know city codes and whether you would need a permit? If so, include it, that is valuable.

Best of luck.
I think that what you have is well thought out and presented. A suggestion I would make is to re-word the second paragraph to utilize keywords such as "requirements gathering", "quality control", "client satisfaction", "design specifications".
