My wife did a consulting job last year. What form do I need to fill in for Tax purpose? for SS? for medicare?

A schedule C or C-EZ to show the consulting income and any associated expenses, then a schedule SE to calculate self-employment tax which is for social security and medicare. The numbers from the bottom of both of these schedules will transfer to the 1040.
